How to make claims
The Step description to be changed as follows :

Step 1

A claim must be notified to the Family & Medical Takaful Claims Department in writing immediately after the event.

Inform Takaful IKHLAS by contacting our Customer Service at 03-2723 9999 (office hours) or go to the nearest branch and fill the claims forms provided in the website.

Step 2

The claim form must be completed and signed by the Claimant/Participant.

As for Death claim, the claim form must be completed and signed by the rightful claimant to whom the benefit is payable in the following manner:-

  • Wasi - The statement must be completed and furnished by the named person in the wasi form.

  • Beneficiary who is nominated under conditional hibah – The statement must be completed and furnished by person(s) whom the benefit is to be payable. For minor cases, the statement must be completed by his parents or legal guardian.

  • Assignee – The statement must be completed and furnished by Assignee.

Step 3

Obtain and forward all the necessary documents to support the claim to the Family & Medical Takaful Claims Dept.

Step 4

All photocopies of documents to support the claim must be certified as true copy by a Commissioner of Oath or Branch Manager or any of Family & Medical Takaful Claims personnel.

For Medical Claim reimbursement, original bill and receipt is required for our processing.
